Show moreBackground and Purpose. Pilates exercises are built around a foundation of movement principles that encourage correct body alignment and biomechanics, proper breathing techniques, efficiency of movement and core stabilization. There are hundreds of different exercises that can be performed on a mat or on the Pilates equipment. Due to the intricacy of the exercises it is imperative that students seek their Pilates instruction from a qualified, certified Pilates instructor. It was the purpose of this study to examine if Pilates students could achieve one of the benefits of performing Pilates exercises, improved postural awareness, by participating in Pilates classes taught by a certified Pilates instructor. Subjects and Methods. Participants of the study include 22 Pilates students and 2 Pilates instructors. Of the 22 participants, 19 were female. All but 2 participants were between the ages of 31-60. The Pilates Perception Questionnaire (PPQ) and the Self-efficacy for Exercise Behaviors Scale (SEEBS), was distributed to Pilates students currently participating in Pilates classes. Items on the PPQ assessed the participants' perception of how they move, their postural awareness and core strength and the instruction they received in their classes. Items on the SEEBS assessed the participants' self-efficacy and exercise behaviors. Descriptive statistics and correlation studies were performed on the participants' demographic information and their answers to the questionnaires. Results. Reports on the Pilates Perception Questionnaire demonstrated a strong correlation (r=.712) between answers given on questions related to postural awareness and instruction. Discussion and Conclusion. Based on the findings of this study the participants perceived that they had achieved improved postural awareness by participating in Pilates classes taught by a qualified, certified Pilates instructor.

A single subject case study design was used. The search for subjects, aged 20-60 years, was conducted throughout the Florida Gulf Coast University population. Exclusion criteria: pregnancy, low back pain, abdominal pain, abdominal surgery in the past year, neuromuscular or autoimmune disease, and those currently exercising. The subject participated in a Pilates exercise routine consisting of a 55-minute session, 2 times each week for a total of 8 weeks, directed by a certified Pilates instructor. Measurements were taken prior to the intervention, at 4 weeks, and following 8 weeks. The Straight Leg-Raising Test and the Sit and Reach Test were used to measure hamstring flexibility. The Partial Curl-up Test was used to determine abdominal muscle endurance. Digital photographs of the subject were taken in standing in the sagittal and frontal planes to examine posture. To assess self-esteem, the Rosenberg Self-Esteem Scale was administered and the subject kept a journal. Findings indicate that participation in a Pilates exercise program may improve abdominal muscle endurance, hamstring flexibility, posture, and self-esteem. There is a need to conduct more controlled research using Pilates' methods because Physical Therapists are incorporating these methods into their treatment plans.

Show moreIntroduction: This case report involves a female with low back pain who was treated with a modified Pilates Method exercise program combined with specific interventions. The hope is to provide a template for a comprehensive treatment plan for chronic low back pain that includes a Pilates based program. Case description: The patient is a 66-year-old female referred to physical therapy for back pain. Systems review The patient presented with decreased active and passive trunk flexion. Her hamstrings were tight bilaterally. The deep stabilizer muscles of her core and back were weak and palpation revealed a tight piriformis in her left hip. Clinical impression/diagnosis: It was believed patient’s pain was a result of abnormal posture, tight musculature, and poor core strength. X-rays supported this diagnosis. Prognosis: The patient was determined to be a good candidate for a combines Pilates lumbar stabilization program. Plan of Care: The goals included the ability of the patient to demonstrate neutral spine posture, restore normal flexibility to hip and knee musculature, increase core and lower extremity strength to normal, to ambulate at least 30 minutes outdoors and return to golf and exercise with a pain rating of <2/10. Interventions: The patient was progressed through 3 stages of Pilates rehabilitation. Sessions included manual stretching of bilateral lower extremities with a focus on pelvic balancing, myofascial release of her low back, deep tissue massage, posterior/anterior joint mobilizations and the use of microcurrent point stimulation. Outcomes: After 18 visits the patient reported no pain and goals were met. Discussion: The patient was seen several months later and was still pain free indicating the success of a combined approach to the treatment of chronic low back pain for a patient with lumbopelvic instability.

Show moreBackground and purpose: Cancer is one of the major leading causes of death in the United States and continues to be a prevalent pathology encountered by physical therapists today. Patients diagnosed with breast cancer will undergo multiple combinations of treatment methods that lead to a myriad of side effects involving multiple body systems. This case report describes the use of mat based Pilates exercises as part of the physical therapy management of a patient with breast cancer related shoulder impairments. Case description: The patient was a 76-year-old female treated in an outpatient physical therapy setting following the diagnosis of stage III breast cancer with subsequent bilateral mastectomy, chemotherapy, and radiation therapy. The patient presented with left upper extremity pain, upper extremity mobility impairments, and bilateral chest wall pain. Outcomes: Physical therapy treatment consisted of manual lymph drainage (MLD), manual therapy techniques, modalities for pain relief and promotion of tissue healing, patient education for independent lymphedema management, establishment of a home exercise program, and therapeutic exercises comprising of mat based Pilates. The exercise program was created specifically for the case patient through collaboration by a physical therapist and a Pilates certified instructor. Studies have shown that performing Pilates exercises is beneficial to improving the side effects of secondary lymphedema in addition to improving shoulder range of motion and strength in patients with breast cancer related deficits. Following physical therapy intervention, the patient demonstrated improved pain levels on the visual analog scale (VAS) from 4/10 in the right chest/shoulder to 2/10 and from 7/10 in the left chest/shoulder to 4/10. The patient also improved their overall upper extremity function as noted by a score increase on the Quick DASH from 52 to 36 and active range of motion (AROM) improvements with right shoulder flexion, left shoulder flexion, and left shoulder abduction. Discussion: This case report demonstrates the potential use of mat based Pilates exercises as a viable exercise method in the multi-modal treatment approach for patients affected by breast cancer related shoulder deficits. The exercise method has also shown to be safe and beneficial to a patient who was currently receiving active radiation therapy as part of their cancer treatment.
